Overview of the Recovery Process

Microsurgical varicocelectomy is a minimally invasive surgery and most patients can return home on the same day. While the full recovery process varies from person to person, most patients can return to their normal daily activities within 1-2 weeks.

Improvement in sperm quality requires a longer process; it usually becomes evident within 3-6 months. This period reflects the time required for new sperm cells to mature (the spermatogenesis process takes approximately 72 days).

Day-by-Day Recovery Schedule

Surgery Day (Day 0)

You will be kept under observation for a few hours after surgery. You can return home on the same day with a companion. You may feel slight nausea and numbness. Consume plenty of fluids and rest.

Days 1-2

Mild to moderate pain is normal. Use prescribed painkillers regularly. Ice application (20 min on, 20 min off) reduces swelling. You can take short-distance walks.

Days 3-5

Pain decreases significantly. You can return to desk work. You can take a shower, but do not rub the wound area. Continue using supportive underwear.

1 Week

First follow-up examination. Sutures are checked (if dissolvable sutures were used, they do not need to be removed). Swelling and bruising have largely subsided. Return to normal activities begins.

2 Weeks

Sexual activity can be started. Light physical activity and walking can be increased. Most daily activities normalize.

4-6 Weeks

Full physical recovery. Heavy lifting and intense exercise are now safe. Can return to all sports activities.

3-6 Months

Semen analysis check. Improvement in sperm parameters is evaluated. Guidance is provided for pregnancy planning.

Post-Surgery Care Recommendations

Ice Application

Apply ice for the first 48 hours in a 20-minute on, 20-minute off cycle. Apply over a thin cloth, not directly to the skin.

Supportive Underwear

Wear tight, supportive underwear for at least 2 weeks. This reduces swelling and minimizes discomfort.

Medication Use

Use painkillers as prescribed. Even if the pain is not severe, regular intake is recommended for the first 2-3 days.

Things to Avoid

Avoid heavy lifting, intense exercise, and standing for a long time for 4-6 weeks. Alcohol and smoking delay healing.

Return to Activities

Activity When? Notes
Shower After 24-48 hours Do not rub the wound, dry gently
Bathtub/Pool After 2 weeks After the wound has closed
Desk Job After 3-5 days Avoid sitting for long periods
Physical Work After 2-3 weeks Start slow, do not force
Light Walking After 1-2 days Short distances, at home
Sexual Activity After 2 weeks Can start if there is no pain
Light Exercise After 2-3 weeks Walking, light jogging
Heavy Exercise/Sports After 4-6 weeks Weights, intense cardio
Driving After 1 week Do not sit for long periods

When to Consult a Doctor?

While some complaints after surgery are normal, consult your doctor immediately in the following cases:

Emergencies
  • Fever over 38°C
  • Increasing redness, swelling, or discharge in the wound area
  • Severe pain that continues to increase
  • Excessive swelling or hematoma (bruising)
  • Difficulty urinating
  • Nausea and vomiting (after the first 24 hours)

Improvement in Sperm Quality

The main purpose of the surgery is to improve sperm quality. This process requires patience because sperm cells spend approximately 72-74 days maturing in the testicles.

Control Semen Analyses

  • 3rd month: First control semen analysis
  • 6th month: Second control, more significant improvement expected
  • 12th month: Maximum improvement is usually seen in this period

Expected Improvement

  • Improvement in sperm parameters in 60-80% of patients
  • Average 2-3 fold increase in sperm count
  • 40-60% increase in motility
  • Reduction in DNA fragmentation
Chance of Pregnancy

The natural pregnancy rate within 12 months after surgery is between 30-50%. The need for IVF or ICSI may decrease, or the success rates of these methods may increase.

Frequently Asked Questions About the Recovery Process

Since you are under anesthesia during surgery, you do not feel pain. There may be mild to moderate pain for 2-3 days after surgery, which is controlled with prescribed painkillers. Most patients become pain-free after 3-5 days.

Yes, mild swelling, bruising, and a feeling of sagging in the testicle are considered normal in the first weeks after surgery. These findings usually resolve within 2-4 weeks. Using supportive underwear reduces this discomfort.

Improvement in sperm quality begins to be seen within 3-6 months. The natural pregnancy rate within 12 months after surgery is 30-50%. Sexual activity can be started after 2 weeks, but waiting 3-6 months for pregnancy is recommended.

The recurrence rate after a correctly and completely performed microsurgical varicocelectomy is less than 1%. In surgeries performed inadequately, varicocele may continue and corrective surgery may be required.
